Your Visit

Thank you for choosing Puyallup Surgical Consultants a division of Proliance Surgery. Our health care team is committed to providing you and your family with the best healthcare possible. When arriving at our clinic for an appointment please remember to check in about 15 minutes before your scheduled visit. This will allow our staff to ensure you have filled out all the proper forms and paper work your healthcare provider may need.

In order for us to provide the best care for you, it is very important that you bring the following items to your visit:

  • Valid photo ID
  • Insurance information and card
  • A list of your current medications
  • Any and all x-rays films
  • Information about what treatments and medications have been tried in the past
  • Medical records from prior visits that you think may be helpful

Your Examination

Your examination will begin with the nurse or medical assistant taking your vital signs. In preparation of your exam you will be asked to change into a hospital gown. Your surgeon will inquire of the exact nature of your problem, perform a physical examine, review your records, laboratory studies, and x-rays to determine the procedure that is right for you.

If you do require surgery, the type of surgery, recovery, complications, and outcomes will be discussed with you at this time. The surgeon will also discuss instructions regarding your preparation for the specified procedure. Follow-up Visits

Postoperative visits are scheduled as needed. Postoperative exams are generally fairly quick as long as there are no ongoing issues. Surgery and recovery results are reported back to the referring physician.

You may be asked to complete various forms, including several documents releasing information, and an extensive form requesting information regarding various aspects of your medical condition. This is vitally important for us, so that we may best provide for your surgical care in a safe and responsible manner.
