Getting Ready for Your Visit

Your First Visit

When you arrive for your first visit, please try to be on time, as that will influence how quickly the physician can see you. We typically are rarely more than fifteen minutes behind the schedule. Special circumstances may influence how quickly you may be seen. Please allow yourself some additional time to complete forms and questionnaires.

In order for us to provide the best care for you, it is very important that you bring the following items to your visit:

  • A list of your current medications
  • Any and all x-rays films
  • Insurance information and card
  • Information about what treatments and medications have been tried in the past
  • Medical records from prior visits that you think may be helpful
  • New Patients: download a patient registration below

Download Patient Forms

You will be asked to complete various forms, including several documents releasing information, and an extensive form requesting information regarding various aspects of your medical condition. This is vitally important for us, so that we may best provide for your surgical care in a safe and responsible manner.

Your Examination

Your examination will begin with the nurse or medical assistant taking your vital signs, and having you un-dress as appropriate for the examination that you will need. Your surgeon will first inquire of the exact nature of your problem, and then examine you and your laboratory studies and x-rays to determine the nature of surgery that will be needed.

If you require surgery, the type of surgery and its recovery, and the possible types of problems that you might have will be discussed with you. You may be able to find much of that information on this webpage by looking either under your disease or ailment, or looking under the procedure being performed for you. Instructions regarding surgery will be discussed with you at that time.

Follow-up Visits

Follow-up visits are to be scheduled by you following surgery. We try to see everybody who has had surgery, to make sure that there are no wound problems or other ongoing problems. This visit should be fairly quick if there are no ongoing problems. We will always report back to your referring physician to let them know about the nature of your surgery and recovery.
