Office Policies
Medical Records
Your medical record is confidential. You must sign for consent for its release. This means that we will not release your records to family, other physicians, legal entities, or even insurance companies unless you have given us signed permission to do so.
Financial Policy
Puyallup Surgical Consultants requires payment in full, or insurance assignment from all patients. PSC will collect co-payment, co-insurance and deductible amounts, in accordance with the contracts we have entered into as a preferred provider. Patients should contact their health insurance carrier for determination of benefits prior to your appointment, procedure or surgery.
As a service to our patients, PSC will complete and submit all insurance forms on behalf of patients with prior written authorization of the patient. We do not however, accept responsibility for collecting insurance benefits, or becoming involved with disputed claims.
Each patient must be prepared at the time of service to fulfill their financial obligations to PSC, and should be prepared to provide all health insurance cards and information. Specific coverage issues may be addressed by your employer, your plan administrator or your insurance carrier.
Prescriptions
We will handle prescriptions during our regular office hours except in emergencies. Please try to anticipate your needs and call for refills before running out. NO narcotic prescriptions will be refilled after hours.
Refilled after-hours/weekends/holidays
When the office is not open, your doctor may be reached for urgent concerns through the office phone number and our voice mail system. Listen carefully to the instructions you are given, and please allow twenty minutes for the doctor to respond. If one of the surgeons is out of town, another will be covering and able to respond to emergencies or concerns that you might have.
Emergencies
Our surgeons are often called out for emergencies and this can cause delays or necessitate cancellations of office visits on short notice. We appreciate your understanding of this matter. If you should have a urgent emergency and cannot immediately reach the doctor, please either call 911 or go immediately to the emergency room.
Cancellations
If you will be late or must cancel an appointment or surgery, we ask that you notify us as soon as possible.
